There are two versions of the form, depending on your certifications. There is one form for UMC-Ordained Ministers and another form for anyone else maintaining Global Membership and certifications.

Your form can be filled out, signed, and submitted electronically by e-mail (instructions provided below). If you prefer, you can print out the document to complete, sign, and mail to us. As always, you may pay any applicable renewal fees by credit card (call UMC office) or by check (mail). Upon receipt of all pending renewal forms and payments, a current renewal validation sticker will be sent to you to update your membership card.

Note: If you wish to sign and submit your form electronically, you must download and save the form to your device before filling it out. The electronic signature will not work on forms opened directly from this page.
